6 Fertilizer will restore 1% of a farms fertility.  Given that current crops require 6 months to grow you need to provide enough fertilizer each month to match each crops fertility penalty.  For instance 10 fertilizer per month is needed for Potatoes, 15 fertilizer per month is needed for Corn, and 12.5 fertilizer per month is needed when you rotate the two crops.
